The pendant features a silver satin finish oval with the figure of Mary in the center. Surrounding Mary are the words, "O Mary, conceived without sin, pray for us who have recourse to thee." On the back of the medal in a brushed finish is the letter M, representing Mary, surmounted by a cross representing the sacrifice of Jesus, and surrounded by twelve stars that represent the twelve apostles.
